As part of this, our community (physical health) services in Bury, Oldham, Rochdale (adult services) and Trafford will be moving to a new NHS provider in 2019. For Bury, Oldham, Rochdale (adult services), this will take place on the 1st July 2019
We have been very proud to run community (physical health) services over the last seven years, but we believe there are other provider organisations better positioned to enable these community services to achieve more for the benefit of local people. We are working closely with our staff and colleagues in the new provider organisations to ensure the smoothest possible transfer.

The Be Well Service includes Health and Wellbeing Advisors, Senior Health & Wellbeing Advisors, Health Check Workers, Health Champions, Community Liaison Workers, and an Oral Health and Training Development Team. You would be joining a service that is dedicated to improving the health and wellbeing of our local communities and this job provides a great opportunity for you to develop skills and knowledge of working in this interesting and challenging field of work, and ultimately helping to make a real difference to people’s lives in Tameside.
We are looking for someone to become part of our management team to provide operational and performance management for a locality team coordinator and provide support and guidance to other staff working on key areas of work which may include community liaison, training, smoking and tobacco/nicotine obesity, oral health, nutrition, physical activity and mental wellbeing.
You would take a lead in one or more of the areas listed above and represent the service at strategic meetings locally or regionally as appropriate to build and sustain relationships with partner organisations to identify opportunities for joint working, increase reach and referrals and maximise positive outcomes for clients.
As well as being part of a great team, we have a well-structured support system including an intensive and comprehensive induction programme, robust individual supervision, group meetings and continuous support, personal development plans and access to a range of internal training opportunities which gives scope for professional development.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
