Service/Ward/Department Summary:

An exciting opportunity has become available to work within the Early Intervention Team at Rochdale HMR.

Role Summary:

We are looking to recruit an experienced Nurse to undertake the role of Physical Health within our service, this is a developing role and will be ensuring that we are promoting good physical well-being for our service users.

You will be able to work autonomously and as part of a MDT, with a broad knowledge of mental well being and the impact this has on physical Health.

Key Responsibilities of the Post: 

To participate and provide clinical sessions. Ensure physical health assessments and interventions are completed for service users identified in high risk groups in line with associated key performance indicators, e.g. CQUIN targets etc.

To take a lead role in the evaluation of physical healthcare guidance and to translate this into practice to ensure the service maintains excellence in practice 

To take a lead role in supporting the senior management team and team managers with the embedding of any new physical healthcare policy or practice 

To champion the Physical health and wellbeing agenda within the community team 

Act as a physical health practitioner and advice resource for care coordinators

Key Requirements of the Person Specification:

Registered Mental Health Nurse (RMN) or Registered General Nurse (RGN)

Evidence of post registration qualifications or training

Substantial experience of working as an autonomous clinical practitioner

Experience of working with mental health patients in any health / social care setting

You must have appropriate UK professional registration.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
